Discussed/menus not available, at the printer. Has applied for two variances. Hep A available was ok. Provide more updated records for next routine inspection.

The Tavern, 13659 Big Bend, Valley Park
Inspection Date: July 11 (complaint)

Complaint Narrative: Complaint received via email, contents pasted below. "I would like to report some health concerns taking place at the tavern kitchen and bar at the corner of Dougherty ferry and big Ben. I have been working there for some time and I have noticed many things. First the hand washing sinks are always blocked and filled with something. They place the food they are preparing on the shelves with the clean dishes. No one ever washes their hands. There are people eating in the dish room dairy product are always sitting out. Pan are usually still wet when they stack and use them. Sanitizer is never checked. They soak dishes in the floor mopping bucket." Location: Establishment. Observation: Establishment appears to be closed, tables pushed to one corner, bar emptied, left card for establishment to call with operating status. Action: Will monitor status. Outcome: Complaint will be closed.

Raw eggs stored over box of shredded cheese in walk-in cooler. Raw animal products shall be stored below ready to eat foods. COS moved. Can opener blade soiled with debris. Food contact surfaces shall be clean to sight and touch. COS cleaned. Cheese slices and cheese crumbles at 51F in cold top unit across from cooking equipment. Cold holding shall be 41F or below. COS discarded all PHF from cold top and start fresh for dinner service. Several gaskets broken on refrigeration units in kitchen. Lid for cold top broken from hinges. All equipment shall be in good repair. Correct by 7-28-13. Gaskets heavily soiled with food debris and black mold like substance. Non food contact surfaces shall be cleaned at a frequency to prevent buildup. Correct by 7-28-13. Mop sitting in mop bucket not being used. 6-501.16 Mops shall be allowed to dry when not in use. Correct by 7-28-13. Prep area walls and walls by dish area soiled with food debris. Facilities shall be cleaned as often as necessary to keep clean. Correct by 7-28-13.

Caulk behind handsink and 3 compartment sink damaged. Fixed equipment shall be properly sealed. Correct by 7-28-13. Accumulation of ice debris in walk in freezer down stairs. Walk in cooler floor upstairs soiled, especially under storage racks. Non food contact surfaces shall be cleaned at a frequency to prevent buildups. Correct by 7-28-13. Gaskets on reach in of line/prep area is broken. All equipment shall be in good repair. Correct by 7-28-13. 43 Sprayer arm broken causing constant leak. Plumbing shall be in good repair. Correct by 7-28-13. Lights above storage racks next to walk in refrigerator are out. Lighting intensity shall be adequate. Correct by 7-28-13.
